Which documents do I need to apply for a visa ?
The documents which you have to submit with your visa application depend on the
reason of your visit. Please find below the checklists of different categories.
For accompanying applicant(s) planning to travel with an applicant applying under
Business or Car Buyer Categories, etc., it is important that such accompanying
applicants should apply under Tourism Visa Category and provide the required
set of documents accordingly. This is also applicable to immediate family
members of an applicant who applies for Business or Car Buyer visa and other
family members desire to accompany him/her.
Please note:
Effective December the 1st the new requirement entered into action for minors who are not accompanied by both parents: original notarized parental consent, original birth certificate, passport photocopy of both parents must be submitted at the time of the application submission.
In case doubts have arisen about the authenticity of documents they will be submitted for further investigation. Duration of this investigation may take several weeks. If forgery or counterfeit is determined, the document will be forwarded to the competent authorities. The same applies to fraudulent obtained documents.
The withdrawal of documents from legal proceedings is a de facto act against which legal objection is not possible.
Passport requirements:

An international passport or official travel document which is valid for all Schengen countries should be valid for at least another three months after departure from the territory of the Schengen countries and contain at least 2 blank pages to affix the visa.

Non-Ukrainian applicants must submit their Ukrainian residence permit. This residence permit should be valid for at least another three months after departure from the territory of the Schengen countries. Please also provide a photocopy of your residence permit.
